A retrospective review of serological response to the 2.0 ml intramuscular four doses schedule of hepatitis B vaccine in chronic kidney disease patients

Background and Aim: Chronic kidney diseases (CKD) patients, particularly those on hemodialysis, are at high-risk to acquire hepatitis B virus (HBV) infection. They vaccinated with four doses, 2.0 ml each, of HBV vaccine 0, 1, 2, 6 months. However, Indian data limited vaccine-induced seroconversion in CKD patients. We retrospectively reviewed the response their sustenance four-dose schedule population. Methods: included unvaccinated adult patients glomerular filtration rate (GFR) < 60 ml/min. Those C or HIV coinfection, post-organ transplant, presence malignancy, immunosuppressive medication were excluded. Anti-HBs antibody titers measured after one month fourth dose followed by once a year thereafter. used two different types quantitative assay, depending upon availability. The limit detection for assay was 3-500 mIU/mL 3-1000 mIU/mL. Antibody reported as >500 >1000 counted 500 1000 respectively. Seroconversion defined detectable anti-HBs antibodies seroprotection titer ≥10 Results: 208 (71.9% males; age 44 [33-55]; stage II/III/IV/V 1.4%/7.2%/26.4%/64.9%; 46% hemodialysis). 124 (12-500) 3 (2-8) months dose. seen 174 (83.7%) 328 (28-500) mIU/mL; 161 them (92.5%) had achieved seroprotection. Overall, (77.4%) median levels ≤6, 7-12, 13- 24 doses 116, 478, 43, 70 mIU/ml Age, BMI, CKD, serum albumin, dialysis status no impact (p<0.05) response. Conclusion: population shows good sustained responses 4 regimens vaccine.
